Patellofemoral disorders observed in athletes encompass a wide spectrum, ranging from patellofemoral pain to patellofemoral instability. Due to the lack of a consensus in diagnostic and treatment methodologies, patellofemoral problems have been referred to as `the black hole of orthopedics`. A significant portion of patellofemoral issues are attributed to overuse syndromes. Typically, patellofemoral disorders respond positively to short periods of rest from sports activities, physical therapy, resistance exercises, and nonsteroidal anti-inflammatory medications. Only a small fraction of patients with patellofemoral disorders require surgical intervention. Patients with dislocated osteochondral or chondral fractures following patellar dislocation, recurrent patellar dislocation, or those with patellar osteochondritis dissecans may benefit from surgery. In this review, patellofemoral problems causing anterior knee pain in athletes have been reviewed.
