Rehabilitation following revision anterior cruciate ligament (ACL) reconstruction requires a more comprehensive clinical decision-making process than rehabilitation after primary ACL reconstruction and should be individualized using criterionbased progression. This review summarizes current evidence regarding rehabilitation after revision ACL reconstruction. Preoperative rehabilitation, phase-based postoperative rehabilitation protocols, progression criteria, return-to-sport readiness assessment, and secondary injury prevention strategies are comprehensively reviewed. In addition, rehabilitation considerations for pediatric and adolescent patients, female athletes, elite athletes, and recreationally active individuals are discussed. The recommendations presented in this review are based on a synthesis of recent systematic reviews, meta-analyses, international consensus statements, and clinical studies focusing on revision ACL reconstruction. Current evidence supports progression based on objective clinical criteria, including pain, joint effusion, range of motion, muscle strength, neuromuscular control, functional performance, and psychological readiness, rather than time alone. A structured, individualized, and criterion-based rehabilitation program is essential for optimizing functional outcomes, facilitating safe return to sport, and reducing the risk of reinjury following revision ACL reconstruction.
