Rehabilitation approaches in lengthening surgery

Limb lengthening surgeries are frequently utilized, effective methods for treating orthopedic problems such as leg length discrepancies, congenital deformities, and post-traumatic conditions. Initially performed using the Ilizarov external fixator system, these procedures have evolved over time with the development of modern techniques such as lengthening over nail (LON) and magnetically controlled expandable nails (Precice®, Ellipse Technologies, Inc., Irvine, San Diego, CA), which aim to improve patient comfort and reduce the risk of complications. Each method has its own advantages and limitations, and the selection of the appropriate surgical technique is determined based on the patient`s underlying diagnosis and the surgeon`s preferences. However, regardless of the surgical method chosen, rehabilitation is considered an essential and critical component of the treatment process. Rehabilitation encompasses both preoperative and postoperative phases. The preoperative phase includes patient and family education, musculoskeletal assessments, and individualized exercise planning. The postoperative phase focuses on pain management, preservation of joint range of motion, strengthening of muscles, balance and proprioception training, and proper positioning strategies. Structured rehabilitation protocols aim to prevent common postoperative complications such as pain, joint stiffness, contractures, and deformities, thereby enhancing the patient`s functional capacity.
