Robotic total knee replacement surgery with ROSA® knee system and basic principles

Total knee replacement (TKA) is an effective surgical method commonly used in the treatment of osteoarthritis. However, patient dissatisfaction remains at rates around 15-20%. Although mechanical alignment is traditionally preferred, it can create a non-anatomical knee structure. Kinematic alignment, on the other hand, aims to recreate the natural knee anatomy, minimizing the need for soft tissue release. Robot-assisted surgical systems help surgeons optimize their intraoperative decisions. The ROSA® knee system works with patient-specific anatomical data to adjust implant placement with millimeter precision. This system can offer better mechanical axis alignment, soft tissue balance and predictable postoperative outcomes. During surgery, femoral and tibial reference points are determined, bone incisions are robotically planned and stability ligament balance is assessed. With ROSA®`s validation tool, planned and realized incisions are compared. With personalized alignment, the patient`s natural ligament tension can be preserved, resulting in better functional outcomes. Recent studies show that this method, in combination with medial congruent implants, offers faster healing and high patient satisfaction. Robotic-assisted knee replacement surgery is an innovative approach that promotes long-term functional success by improving patient satisfaction through the advantages of precision and personalized alignment. In this article, we have tried to write about how the ROSA® knee system works in general terms, its working principles and how it helps the surgeon in which steps.
